Transgender and Non-Binary Breast Health: Understanding Mammogram Needs and Options

In honor of Pride Month, we’re shining a light on an essential aspect of healthcare: mammograms for transgender and non-binary individuals. Breast health is vital for everyone, and understanding your options can make navigating this aspect of your wellness journey a breeze. At Akira Medical Imaging + Wellness, we celebrate diversity, inclusivity, and the unique health journeys of every individual!

Why Mammograms Matter for Transgender and Non-Binary Individuals

Breast cancer doesn’t discriminate, and everyone with breast tissue should be aware of their breast health. Regular mammograms are important for early detection, which significantly improves treatment outcomes. Whether you’re transgender, non-binary, or anywhere on the gender spectrum, here’s what you need to know:

Transgender Women (Male-to-Female):

  • Average-Risk with 5+ Years of Hormone Use: If you are 40 years of age or older and have been on hormone therapy for five or more years, digital breast tomosynthesis (DBT) or a screening mammogram may be appropriate.
  • Higher-Risk with 5+ Years of Hormone Use: DBT or a screening mammogram is usually appropriate for those at higher risk and aged 25 to 30 years or older.
  • Little to No Hormone Use (Less Than 5 Years): No screening is necessary for average-risk individuals. However, for higher-risk individuals aged 25 to 30 years or older, DBT or screening mammograms may be appropriate.

Transgender Men (Female-to-Male):

  • Bilateral Mastectomies (Top Surgery): No screening is necessary for individuals with bilateral mastectomies, regardless of age or risk level.
  • Average-Risk with Breast Reduction or No Chest Surgery: DBT or a screening mammogram is usually appropriate for women 40 or older.
  • Intermediate-Risk with Breast Reduction or No Chest Surgery: For those aged 30 years or older, DBT or a screening mammogram is usually appropriate, and a breast ultrasound or MRI with and without intravenous contrast may be appropriate.
  • Higher-Risk with Breast Reduction or No Chest Surgery: For individuals aged 25 to 30 years or older, DBT or a screening mammogram or MRI with and without contrast is usually appropriate, and a breast ultrasound may be appropriate.

While these are just guidelines, we recommend discussing these guidelines with your provider to tailor a plan to your needs to ensure you are receiving appropriate care.

Navigating Gender-Affirming Healthcare Environments

We understand that the thought of a mammogram can induce an array of emotions and feelings, especially if you’ve had negative experiences in healthcare settings. Here at Akira Medical Imaging + Wellness, we strive to create a welcoming and affirming environment for everyone. Here are some tips to help you navigate your mammogram with confidence:

  1. Find an LGBTQ+ Friendly Provider: Look for healthcare providers who explicitly state their commitment to LGBTQ+ inclusivity. At Akira, we’re proud to offer gender-affirming care and have trained our staff to understand and respect your unique needs.
  2. Communicate Your Needs: Don’t hesitate to communicate your pronouns, name, and any specific concerns about the procedure. A good provider will listen and adapt to ensure your comfort.
  3. Consider Your Comfort: If you’ve had top surgery, let your provider know; they may need to adjust the mammogram technique. Wearing comfortable clothing and bringing a friend or support person can make the experience easier.
  4. Ask Questions: If you’re unsure about anything, ask! Whether it’s about the procedure itself or how often you should get screened, your provider is there to help you feel informed and empowered.
